KINDERGARTEN REGISTRATION SET
Kindergarten registration in Hamilton
County, for children who will be five (5) years of age on or before September
1, 2010, has been scheduled for the following dates and times:
Dahlgren Monday, February 22, 2010 from 3:30 -
5:30 p.m.
East Side Tuesday, February 23, 2010 from 3:00 –
5:30 p.m.
or
Wednesday,
February 24, 2010 from 3:00 – 5:00 p.m.
A. Parents must bring the following to
registration:
(1)
Child’s legal birth certificate (not the hospital certificate)
(2)
Immunization Records (showing the day, month, and year of each
inoculation)
(3)
Social Security Card
Note: If your child is enrolled in Hamilton County
Pre-School, you will only need to bring the child’s social security card. All pre-school health records are on file.
State law requires that all children
entering school in Illinois, or prior to admission, shall present evidence of
immunity against: Diphtheria, Pertussis,
Tetanus, Rubeola (measles), Poliomyelitis, Rubella, Mumps, and Varicella. DPT, Polio, and MMR boosters after age 4 are
required to enter kindergarten. Hepatitis
immunizations are recommended. Appointments
for shots through the Hamilton County Health Department can be made at that
time.
NOTICE: Parents of
students who live on current Dahlgren bus routes or in Dahlgren should register
their child at Dahlgren School; all other parents should register their child
at East Side School. It is anticipated
that where you register is where your child will attend. If you prefer otherwise, you must let the
principal of the building in which you register know in writing on the day of
registration. ALSO, parents who do not
register their child on the designated days may not get their preferred choice
of school attendance center when class size is balanced. Tentatively, there will be three classes at
East Side and one class at Dahlgren. The
administration will be assigning students to buildings and classes in order to
balance classes as best as possible.
B. At the start of school, parents must
provide:
(1)
Completed Immunization and Lead Screening Records
(2)
Physical & Dental Exams (on the correct state school forms) -
completed in entirety.
(3)
An Eye Examination (on the correct state school form).
C. The Southern Illinois District 15 Masons will be present to
create a free “Child Find” packet for your child, if you choose to participate.
It is very important that all kindergarten students are registered
early, so that transportation can be arranged, information regarding health
requirements can be distributed, and class size can be determined.
